This workshop empowers participants to articulate and communicate their end-of-life preferences with clarity and confidence.
Thoughtful planning and open conversations can reduce uncertainty and ensure dignity for both you and your loved ones.
Key Topics Covered:
- Advance Directives: Define your preferences for medical treatments, including those you wish to accept or refuse during critical illness.
- Health Care Proxy: Designate a trusted individual to speak on your behalf if you are unable to do so. Learn how to select and inform your proxy to align with your wishes.
- Organ Donation: Explore the process, implications, and registration requirements for endowing your body or becoming a donor.
- Preparing for Conversations: Use practical tools, such as the Relationship Map and VITAL Matters Cards, to foster meaningful discussions with loved ones, medical practitioners, and others involved in your care.
- Non-Medical Lifestyle Wishes: Create a personalized video message to share your values, priorities, and specific wishes for your final days.
